Notice to the wonderful patients of
Dr. Dustin C. Derrick, MD:
After careful consideration, I am transitioning to a new practice, and as a result, I will no longer be with Fort Worth Plastic Surgery (FWPS) effective July 2, 2026.
I want to express my sincere gratitude for the trust you have placed in me as your surgeon and doctor. I am committed to ensuring a smooth transition of your care and will be at my new office on July 2, 2026.
To help with this transition, fill out a medical release form. The form is to allow transfer of your medical records to the new office. Simply fill out and then either scan or snap a photograph of the completed form and email it to: Info@CosmeticSurgeryofDFW.com. If you were my patient prior to joining FWPS on 8/10/2021, my new practice will be the custodian of your records. If you became my patient on/after 8/10/2021, then FWPS will be the custodian of your records.
If you need follow-up appointments past July 1, 2026 please call 817-529-4314 to set up an appointment with my new practice.
I want to assure you that your well-being remains my top priority, and I am dedicated to making this transition as seamless as possible for you.
